Medicaid Targeted Case Management (MTCM) is a service focused on achieving beneficiary wellness through communication, education, and service identification and referral. MTCM is a time-limited service that provides an organized structured protocol for moving beneficiaries through the process of change and toward the goal of self-sufficiency.

The MTCM process is a shared partnership between the beneficiary and/or responsible party and the case manager.

Beneficiaries and/or responsible parties are actively involved in all phases of the process – assessment, planning, problem-solving, and identification of resources.

MTCM ensures available resources are efficiently accessed and being used in a timely and cost-effective manner.
Who We Serve:

We provide Medicaid Targeted Case Management services to at-risk children, adults with serious and persistent mental illness, at-risk pregnant women and infants, and individuals with psychoactive substance disorder.
Referrals:
Step 1. Complete Referral Application and submit.
Step 2. Securely submit any supporting documentation to referrals@branmarc.org
Step 3. Our clinical team will review all information within 24 hours of submission. A clinical decision will be made only after reviewing all information.
